Something I just can't wrap my head around is the constant indoor smoking by men in China, even in restaurants or lobbies right next to children. It seems so obviously selfish and dangerous.
What's even more baffling is that no one ever says anything. Staff don't enforce 'no smoking' signs, and other patrons just ignore it. Why is there no pushback against something so clearly wrong?

Japan was like that too literally 10 years ago.

You have it backwards. The health costs are pro smoking from a governmental pov due to old age and long term care costs. The math might vary based on retirement age but I think some study ran the numbers and assumed if ppl keeled over sooner it actually worked out better given the number of pensioners.

Also, people do not like confrontation nor do they like taking responsibility out of fear of repercussions.

I have 2 children age 6 and 8. I have politely asked someone to stop smoking because of my kids in elevators kaorou restaurants arcades, etc. when ask politely and the kids are right there they always put out their cigarettes.
My wife’s family smoked like a chimney at one of the family gathering in apartment. I didn’t want to insult her family so I took the kids and left. They were all shocked. My wife got kinda mad about it but I don’t care

1 big reason is the ridiculous revenue of China Tobacco (state-owned). For instance they have revenue of 1.5 trillion yuan ($210 billion) in fiscal year 2023. Big revenue big tax for govt plus a highly valued point: Huge company that employs around half a million workers.
Another is the cig-alcohol culture in Chinese business. Minus one and it's a big deal.
These above is making it very hard for any practical crackdown or strong initiatives for any non smoking enforcement.

I'm from the country with the best healthcare in the world (value for money wise) and the government is pretty strongly anti smoking even though it generates fucktons of tax revenue.
