Worked for a breakfast restaurant. Had the same thing happen, expert it was a couple who definitely lost a couple brain cells. They literally put the dead roach on top of the food like a garnish and said "you better give us 500 dollars or we are calling the health department!". To which the head waiter was like "that''s fine, let me just go check the cameras". Dude then tried to bolt without his girl, but forgot his bag. So he paid anyways and left. Never heard from them.

Similar thing happened at the place I used to work for. A blonde customer complained that there was hair in her meal and demanded an apology and money. The chef was called and he picked up on what was happening. He called all staff to come out and apologize. "We are sorry that your dining experience has been less than stellar and that our  staff has somehow managed to add a blonde strand of hair in your food". The blonde Karen was smug but eventually realise that there were no blonde staff member working at the restaurant. Was glorious to behold.
